Please provide a current Certificate of Insurance (COI) naming DUSTIN KING TRUCKING as Certificate Holder and including a Waiver of Subrogation on all applicable policies, including Workers’ Compensation, Commercial Auto, and General Liability.
Coverage must remain current throughout all work performed as a subhauler, with updated certificates provided prior to policy expiration.
Please provide a current W9 for your trucking company.
Download a copy of our load sheet for use on our next project. All subhaulers must turn in paperwork using a Dustin King Trucking load sheet.
